We ask that you please send these forms in at least two business days before your scheduled appointment. |

| Enrollment Form |
| This form is used if your cat has never had an appointment with Cat Specialist. Please use this form if your cat has no current medical issues that you are aware of, and is coming to see us for routine preventative healthcare. |

| Referral Form |
| This form is used if your cat has never had an appointment with Cat Specialist. Please use this form if your cat is coming to see us because he or she has medical or behavioral issues that warrant our immediate attention. |

| I-131 Patient Information Form |
| This form is used if your cat has never had an appointment with Cat Specialist, has been diagnosed with hyperthyroidism, and you wish to enroll him or her for radioactive iodine treatment to cure the disease. |

| Bed & Breakfast Registration Form |
| This form should be filled out EACH TIME your cat boards with us, regardless how often he or she visits us. |
